TREATING INTRACELLULAR LYME
ZITHROMAX (azithromycin)
This antibiotic is related to erythromycin. When used in combination with
Plaquenil, it can potentially kill spirochetes that have not been able to
be touched by antibiotics before, and therefore can make a big difference
in treating chronic Lyme.
Take one daily for the first week, then raise the dose to two daily.
Take the two together as a single dose one hour before or three hours
after food.
I recommend you take them as soon as you get up in the morning, then
wait one hour before you have breakfast.
PLAQUENIL (hydroxychloroquine)
This quinine based drug can help prevent the antibiotic from being
inactivated before it can kill the spirochete.
Take 3 pills daily as separate doses, with food or milk for 7 days
(loading dose).
Then cut the dose to 2 pills daily.
You will need an eye exam by an ophthalmologist as a baseline, then on a
regular basis while on this medication (at least every six months).
WHAT TO EXPECT WITH TREATMENT:
Plaquenil has anti-inflammatory qualities, and prevents severe Jarisch
-Herxheimer reactions. However, do expect symptoms to flare during the
first two to four weeks. The Herxheimers are often unusual, and involve
mainly neurologic symptoms as well as joint pain. About one third of
patients experience diarrhea during this time. If this happens, stop the
medication for two to three days then resume. If this does not control
the bowels, stop the treatment and call. It is extremely rare for this
combination to cause nausea or heartburn.

Not all MD's agree with the "loading dose" theory as some patients cannot tolerate it more than 1X per day. In some cases, it's easier to start with 1 dose and work up to 2 rather than following this 7 day "loading dose" theory. Also, this same post talked about taking Zithromax on an empty stomach. The new Zith doesn't require this, in fact the packaging states take with or without food. The old Zith required an empty stomach. Cindi

The old Zithromax needed to be taken on an empty stomach. The new one can be taken with food. It's not necessarily a dosing issue. I'm on 250mg. 2x/day and it can be taken with or without food. I thought this was a misprint since zith was one of the first abx I took for Lyme several years back so I checked back with the pharmacy, then the MD. Both explained it's a change in formula that doesn't require an empty stomach anymore. Also, zith has a very long half-life and keeps high blood levels for up to 72 hours after taken. When you have to take it twice a day -- the 12 hours apart rule isn't as important as some other abx's. Just stuff I've learned along the way.
